If you will be utilizing bus transporation, this form must be completed before June 26, 2026. The Terrebonne Parish Transportation Department requires any student that will ride the bus during the 2026-2027 school year to complete this linked form. If you have more than one child riding the bus, you must fill out a separate form for each one.
